All Volunteer staff and Youth Mission Trip applicants must print and complete the forms below. The Authorization & Release form and the Child Maltreatment (CMCR) form must be notarized prior to sending them to the district office. If your church has ran backgrounds on you in the past 12 months, you may send copies of those. If our office is running your backgrounds, we will need the completed and notarized forms in our office no later than July 13th, 2026.
